Minor Work Permit ProcessPrintable

Minor Work Permit

Washington State Laws RCW 26.28.60-> and RCW[ 26.28.70-> direct the Washington State Department of Labor and Industries to write and enforce various WAC's-> in support of Minor workers. For purpose of defining Minor Workers this applies to any minor who is paid by Washington State Youth Soccer, the State Referee Committee, or a local Chapter in performance of doing refereeing or related activities with respect to Youth Soccer.

Required Documents

There are two documents necessary to complete this process:

  1. Proof Of Age

    You can submit a copy of your Birth Certificate, a Drivers License, or a Passport.

  2. Parent/School Permission

    As a Teen worker you must complete the Permission form and have it signed by your Parent/Guardian and, if during the school year, your school. SKCSRA's UBI Number is 601-631-506 .

  • The Minor worker MUST sign the form and date it.
  • The Parent MUST sign the form and date it.
  • The School representative MUST sign the form and date it.

Scan and Upload

Scan your two documents to two separate files and store them on your computer. Then you can UPLOAD them to the skcsra website using Services: Youth Referee

If you do not have a scanner, try one of the following:

  • A friend.
  • An Office services store.
  • Your school.
  • Your parents workplace.

Security

Your documents will ONLY be available for viewing by authorized personal of SKCSRA and YOU. The documents will be stored in a secure location unavailable for general Internet access, except through the SKCSRA Site.
